The Breakouts No One Warned You Were Coming Back

1. The oestrogen-to-androgen shift

As estrogen falls in perimenopause, androgens rise in relative terms. They drive sebum around the jaw and chin, and the oil is thicker, more inflammatory, and more likely to form deep cystic lesions. An endocrine event on your face, not a hygiene failure.

2. Why your twenties playbook hurts you now

Benzoyl peroxide and stripping washes were built for younger, oilier skin. Perimenopausal skin is drier, thinner, and more reactive. Drying it out reduces spots temporarily but destroys the barrier in the long term. Stop stripping. Start supporting.


Quick glossary: BHAs (salicylic) unclog sebum inside the pore. Azelaic acid normalizes the follicle and calms inflammation. Niacinamide regulates oil. Retinoids prevent clogging at the source; use low and slow.

3. The protocol that works

Azelaic acid, morning and evening, is the anchor. A targeted salicylic and tamanu oil, exactly where you need it. A proper evening cleanse to remove the day without stripping.


The bigger lever is hormonal. HRT can calm the system within three to six months. Spironolactone at low doses blocks androgens at the follicle. Doctor conversations, not internet ones.


No toothpaste, lemon juice, or DIY scrubs. Avoid denatured alcohol, mint, and eucalyptus. Double cleanse if you wear makeup. Protect sleep: cortisol is an androgen precursor and will keep the jaw breaking out no matter what you apply.
